To thank you, we are pleased to announce our new patient referral program for a limited time:
Receive a $75.00 credit on your account, redeemable for any services, for each patient you refer who undergoes a new patient exam and cleaning in our office.
Upon arrival, new patients are asked to identify who referred them. Once your name is provided, a $75.00 credit will be applied to your account.
We appreciate your trust and confidence in us, and we are committed to providing the highest level of care and service to anyone you may refer.
Dental Referral Program Rules & Restrictions:
You may receive unlimited credits for patients you refer to our practice; however, those patients must schedule and attend a new patient exam and cleaning at our office.
A maximum of 2 credits will be awarded per family referred (i.e., if you refer a family of 5, you will receive a maximum of 2 credits for that referral).
Thank you for the offer, which does not apply to your immediate family members (spouse and children).
Our new dental referral program starts on October 2, 2014, and is not retroactive.
New patients must mention their referral on the paperwork.
